ACCESS stands for Advancing Chronic Care with Effective, Scalable Solutions. The model began July 5, 2026 and gives Medicare-enrolled care organizations flexibility to deliver clinician-guided, technology-supported chronic care in person, virtually, or asynchronously.

Instead of paying only for a defined list of activities, ACCESS tests Outcome-Aligned Payments tied to the share of patients who reach condition-specific targets. Participating organizations need operations that can keep patients engaged, care teams coordinated, and performance visible over time.
