Outcomes over activities
Organize care around measurable clinical improvement rather than a disconnected series of billable tasks.
DocStation helps ACCESS care organizations and pharmacy partners connect patient operations, clinical workflows, engagement, and reporting in one platform built for care beyond the prescription.
The model
ACCESS stands for Advancing Chronic Care with Effective, Scalable Solutions. The model began July 5, 2026 and gives Medicare-enrolled care organizations flexibility to deliver clinician-guided, technology-supported chronic care in person, virtually, or asynchronously.
Instead of paying only for a defined list of activities, ACCESS tests Outcome-Aligned Payments tied to the share of patients who reach condition-specific targets. Participating organizations need operations that can keep patients engaged, care teams coordinated, and performance visible over time.
Organize care around measurable clinical improvement rather than a disconnected series of billable tasks.
Support a mix of virtual, asynchronous, and in-person care with connected patient and workflow tools.
Create clear documentation, oversight, and coordination paths for accountable chronic care programs.
Build the operational data foundation needed to monitor progress and support model reporting.
Why pharmacy
Pharmacy teams already see patients with cardiometabolic conditions regularly. DocStation helps ACCESS delivery partners make those teams part of a coordinated, technology-enabled care model.
Use the pharmacy's regular patient contact to reinforce care plans, capture progress, and identify follow-up needs.
Bring medication management and clinical service documentation into the broader chronic care workflow.
Coordinate pharmacy work with the participant organization's clinical oversight and referring providers.
Where DocStation fits
DocStation supports the operational workflows around ACCESS care delivery while the participating organization retains responsibility for model eligibility, enrollment, clinical oversight, billing, and compliance.
Keep patient records, services, documentation, and follow-up connected across the care journey.
Translate program protocols into consistent templates, tasks, and team responsibilities.
Capture care activity and patient context in a structured record designed for clinical pharmacy services.
Coordinate scheduling, reminders, and follow-up through connected patient communication tools.
Keep ACCESS workflows alongside existing clinical service and medical billing operations without another disconnected portal.
Monitor operational progress across patients, teams, and locations and prepare data for required reporting paths.
Whether you are an ACCESS care organization or a pharmacy network evaluating a delivery partnership, DocStation can help connect the operating model.
